ATTENTION MEDICAID AND PUBLIC HEALTH INSURANCE RECIPIENTS:
If your residence and/or mailing address has changed over the past two years since the start of the public health emergency, please notify us of this address change. When the public health emergency ends, it will mean that Medicaid renewals will again be mailed to recipients and must be returned to continue coverage.
If your plan is through NY State of Health (the Marketplace) you may report an address change by calling 1-855-355-5777
If your coverage is through a Medicaid case with Oswego County DSS you may report an address change by emailing medicaid@oswegocounty.com or calling 315-963-5463.
