Health Department Announces Closure of Long Term Home Health Care Program

OSWEGO - The Oswego County Health Department (OCHD) has announced it will no longer admit new patients to the Oswego County Long Term Home Health Care Program (Oswego LTHHCP).

The County Health Department is following New York State guidelines and working closely with local health care providers and contractors since receiving approval from the NYS Department of Health to close the Oswego LTHHCP. Four hundred fifty letters were sent to providers and agencies. All of the Oswego LTHHCP patients have been transferred tomanaged long term care agencies or nursing home transition and diversion Medicaid waiver programs. The county Health Department will continue to follow New York State guidelines in the process of closing the county Long Term Home Health Care Program.

Although the Oswego LTCHHP is closing, the Hospice and Certified Home Health Care (CHHA) programs continue to serve Oswego County residents.

"Hospice and CHHA provide skilled nursing care, medical social worker service, and other direct patient care services," said Oswego County Public Health Director Jiancheng Huang. "In these programs we continue to bethe provider of choiceforquality home care services.Hospice and CHHA are the premier home care agencies in Oswego County dedicated to and recognized for our passion and commitment to serve, help, and enhance the quality of life and well-being of others.We are determined to take care of county residents as much as the capacities allow."

As the state-approved plan directs, the public will be informed through the Oswego County website at www.oswegocounty.com and through news releases distributed by the Oswego County Department of Community Development, Tourism and Planning.
